Our Senior Behavioral Health unit, located on the fifth floor of the hospital, is designed to help patients and their families address the complex issues that can cause older adults to fall into a behavioral health crisis. We provide a highly structured treatment that focuses on coping skills, personal responsibility, Individual dignity and self-esteem.
Our Transitions program offers an alternative to Inpatient hospitalization. We provide the transitional step for patients adjusting from hospital care to their return home.
For patients who need or require maximum support during this transition, the Partial Hospitalization Program provides more comprehensive and extended services five days a week. For more stable clients, the Intensive Outpatient Program offers services from one to three days a week, providing a more concentrated therapeutic environment than a traditional outpatient setting.

Transition's behavioral health services are provided by some of the finest health care practitioners in our area. Working hand-In-hand with your doctor, our team f psychiatrists, doctors, social workers, therapists and specialized nurses will provide assessments and treatment plans to meet the needs of the patient and family. As with any other medical program, your right to privacy is always protected.
Additional Services
We offer on-site assessment for local nursing homes, assisted living facilities, hospitals and physician offices. As a convenience, we provide lunch and snacks for patients and transportation if needed. Referrals may be made by a psychiatrist, personal physician, hospital, mental health professional or agency, family member, concerned friend or by self-referral.
