The Diabetes Care at The Medical Center of Southeast Texas is a comprehensive program that provides education on the latest advances in diabetes management to individuals with diabetes, their families and physicians. Our team approach to diabetes care helps people learn the skills necessary to maximize their self-care, to maintain and enhance their quality of life and to avoid diabetes-related hospital stays. We are currently recognized by the American Diabetes Association as meeting the national Standards for Diabetes Self Management Education.

Diabetes Education
A comprehensive course including information, discussion, and problem identification and resolution is offered. This course includes topics such as medication, care of the skin and feet, self-monitoring of glucose, exercise, health and community resources, dental care, and medical nutrition therapy, plus other information essential to your health.

  • Outpatient Individual Instruction (Adults and Children)
  • Preconceptual Diabetes Management
  • Management of Diabetes During Pregnancy
  • Insulin Initiation
  • Insulin Adjustment and Follow-Up
  • Nutrition Counseling
  • Self Glucose Monitoring

Individual instruction is available for persons with existing barriers that impede their ability to obtain skills through group instruction.

Community Presentations, School Presentations, and Healthcare Professional Education are also available upon request.

Classes are held at The Medical Center of Southeast Texas and require a physician referral. To begin the process, please call 409-853-5376. Please allow time for verification of insurance benefits prior to attending the desired class date.

Free Diabetes Support Group is also available. This group meets the second Thursday of each month. Please contact 409-853-5376 to be part of this group and learn how others with Diabetes are living a healthy lifestyle.
