The Medical Center of Southeast Texas maintains medical records for each patient treated at the hospital.

To request a copy of your medical records, please contact our Health Information Management office between the hours of 8 a.m. to 5 p.m. Monday through Friday at (409) 853-5494.

There is a fee for medical records unless the request is for continued care and sent directly to the caregiver. Records needed for continuing care purposes will be sent directly to your provider.

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid government issued photo ID.

If medical records are available, walk-in requests may be picked up immediately.

Requests for medical records may also be made via mail or fax. The Medical Center of Southeast Texas has 15 working days to comply with the requests according to the State of Texas.
